Step 1: Log into CoLab and choose "submit a PDSA" from the jump menu at the top-right of the site. Click ‘Go’:
Step 2: You will be directed to the CoLab PDSA form. At the top of the page, choose the team (1) for which you are completing the PDSA, as well as the change idea (2) that you are working on (if it's not listed, submit a ticket with the change idea that you would like added):
Step 3: Work with your team to fill out your PDSA. At the bottom of the page, you may save the form with the "Draft" submission status (3) and return later to edit or add additional information. When you are ready, change the submission status to "Submitted" and click the "Save" button (4):
